Access to a democratic and efficient health system is still a process under construction, with managers and health professionals playing a key role in its consolidation. To perform their duties, they must combine technical knowledge and the ability to deal with the diversity and complexity of local realities. More skill and competence is required in the application and optimization of public resources, seeking the pursuit of service quality, efficiency and user satisfaction. Understanding today's increasingly complex issues seems to require new research possibilities that allow for multidimensional thinking capable of understanding the breadth of human beings and society at large to meet current challenges. This case describes the management of the Cadernos ESP scientific journal with its challenges and possibilities in the production of scientific knowledge. Cadernos ESP is published by the Cesar Victora Scientific Research Center (CENIC) of the Ceará School of Public Health, an institution linked to the Ceará State Health Secretariat. Created in 2005, it has a semiannual periodicity, and is an instrument of technical and scientific diffusion of inter and multidisciplinary character in various fields of doing health, but especially of Collective Health. Its purpose is to publish works relevant to public health and their interfaces. Throughout these reflections, we note that there are several problems and challenges for the production of knowledge. Learning from the feat enhances us and drives us to desire for the new changes needed to achieve more ambitious purposes in terms of scientific quality, creativity and innovation
